Adult Dependents: Payment Responsibility

The subscriber is responsible for the payment of premiums for adult dependents. The delinquency notice will be sent to the address on the Adult Dependent Enrollment/Change form. If the adult dependent's address is on the Adult Dependent Enrollment/Change (and the adult dependent has a different address from the subscriber) then the subscriber won't receive notices, even if PEBB has the subscriber's address on file for his/her own coverage. 

Rates for Adult Dependents are found on the back of the Adult Dependent Enrollment/Change form.
