You must send a copy of your Medicare card within 60 days of becoming entitled to enroll in Medicare Part A and Part B to:

If you send a copy of your family member’s Medicare card, write your social security number on the copy. We will update your account to reflect the Medicare rate and notify your health plan of your Medicare enrollment.
