If the coverage was applied for within 31-days of eligibility and the effective date is:
- Within the last 3 months, the employing agency may key the correct eligibility and enrollment into the PAY1 system and collect back premiums from the employee.
- Older than 3 months, the employing agency should submit a request for adjustment through FUZE PersPay incident e-mail (www.fuzeqna.com/perspay/consumer/question.asp) and provide documentation of the error or delay.
If the coverage was applied for beyond 31-days of eligibility and the signature date is:
- Within 60 days of todays date, enroll the coverage into PAY1 (pend status) and forward a copy to Standard Insurance Company.
- Older than 60 days, the employee must re-apply by completing a new form. (Standard Insurance Company will not accept applications with dates older than 60 days).
Should an employee disagree with a denial or decision made about benefits offered through PEBB, refer to the Appeals Process for guidance.
