Up to 10 visits per calendar year for manipulations (adjustments) of the spine and extremeties (arms and legs) are covered. When you have reached your 10-visit limit, no further payment will be made.

The following services related to spinal and extremity manipulation are covered under this benefit. A visit that includes these services counts as one visit toward your 10-visit limit, regardless of whether a manipulation is also performed at that time.

• Diagnostic tests
• Office visits
• X-rays

Visits that count toward your medical deductible also count toward your 10-visit limit (see definition of  "Limited Benefit").
