How does UMP cover eye exams, glasses, and contact lenses?

Eye Exams

UMP covers routine vision exams once every calendar year. You don't have to pay the medical deductible before UMP provides coverage for this service. If you see a network provider, UMP pays at the network rate. If you see a non-network provider, UMP pays 60 percent of the UMP allowed charge; you pay the remaining 40 percent, plus any amount that is above the UMP allowed charge.

Glasses and Contact Lenses

For vision hardware—glasses, contacts, and related fitting fees—UMP pays up to $150 per two calendar year period. You don't have to pay the medical deductible before UMP pays. You can buy your vision hardware from any provider; it doesn't have to be a network provider. If you purchase from a vendor who does not submit claims to UMP, it's easy to send in a paper claim.
