Question/Topic
How does UMP cover eye exams, glasses, and contact lenses?
Answer/Article

TIP: Limits to vision care benefits apply per enrollee. Each enrolled member in your family is entitled to one routine eye exam per calendar year, and $150 in vision hardware every 2 calendar years.

This benefit is exempt from the medical deductible and includes:

Eye Exams

The plan covers one routine eye exam for each enrollee per calendar year. The plan pays for these services based on the allowed amount and the network status of the provider.

Hardware (Eyeglasses and Contact Lenses)

The plan pays up to $150 every 24 consecutive months 2 calendar years for prescription eyeglass lenses, frames, contact lenses, and contact lens fitting fees. The plan will not pay more than your actual cost for these items and services. You are responsible for any costs above the $150 limit.

You can buy your vision hardware anywhere; the maximum benefit of $150 applies no matter where you shop. If you go to a provider that does not bill the plan directly, you can submit a claim for glasses or contacts.

If you need to know how much vision hardware benefit you have available, call UMP at 1-800-762-6004, or send us an inquiry.

TIP: If you plan to pay for your eyeglasses or contacts with benefits from this plan and another health plan, call Customer Service first for important information about how the benefits from the two plans work together.
