Where can I get my flu vaccine?

You can go to a UMP network provider for your flu vaccine, or go to one of the pharmacies on the list below that have contracted with UMP to provide flu vaccines and bill UMP directly. You won't have to pay anything upfront.

Or, you can get your flu vaccine at any flu vaccine clinic, then send in the Flu Shot Claim Form PDF to get reimbursed by UMP.
