A maximum of two hours of medical nutrition counseling services are covered when prescribed by an approved provider type for treatment of a covered condition.
Nutrition counseling for weight loss is not covered.
Medical nutrition therapy (formula) is covered only when:
- Prescribed for the treatment of a congenital metabolic disorder (such as phenylketonuria [PKU]);
- Specialized formulas have been established as effective in treating the disorder; and
- Treatment is prescribed by a network provider.
