How does UMP cover insulin and diabetes supplies?

TIP: Enrollees with diabetes may be eligible for Informed Care Management, a free program from ActiveHealth Management; see "What is chronic care management?" for more information.

Diabetic supplies listed below are covered under your plan's prescription drug benefit according to the designated tier in the Washington State Rx Services Preferred Drug List (PDL). To be covered, you must get a written prescription for these medications and supplies. To find out the tier of a product, see the online PDL or call Washington State Rx Services at 1-888-361-1611.

You save money and avoid having to submit your own claims when you purchase these diabetic supplies from a Washington State Rx Services network pharmacy. To find a network pharmacy, check the pharmacy locator, or call 1-888-361-1611.

When covered under the prescription drug benefit, the following diabetes care supplies are covered under the tier listed in the Washington State Rx Services Preferred Drug List:

  • Glucometers
  • Injectable insulin
  • Test strips
  • Insulin syringes
  • Lancets

Continuous glucose monitors must be preauthorized.

If a health plan other than UMP is your primary payer (including Medicare), claims for diabetes care supplies may be paid differently.
