Preauthorization Required for Tier 2 Glucometers and Test Strips

If you have not purchased these items under UMP before, you will need to request preauthorization for glucometers and test strips designated as Tier 2 on the UMP Preferred Drug List.

NOTE: Tier 1 glucometers and test strips do not require preauthorization and are not subject to the prescription drug deductible.
