How does UMP's Medication Therapy Management program work?

UMP is contracting with the Medication Pathfinder Network of Washington for medication therapy management services. Medication Pathfinder will contact enrollees who take multiple (generally 10 or more) prescription medications as part of a review process designed to educate enrollees about the medications they are taking and ensure that all medications are still appropriate for the patient.

The enrollee will be invited to attend a free one-on-one session with a pharmacist. This review is to determine if the enrollee is using the best medications for their condition(s), and to check for use of similar drugs and potential drug interactions. The enrollee and prescribing provider will receive a report and summary of clinical recommendations. This review process is not intended to discourage the use of any particular drug.

When needed, the pharmacist will contact the prescribing provider to obtain additional information, such as lab results.

This program is free and completely voluntary. Please encourage your patients to participate to learn more about their medications.
