I received a letter from the "Medication Pathfinder Network" about my prescriptions and the Medication Therapy Management program; what is this?

UMP is contracting with Medication Pathfinder Network of Washington to help enrollees manage their prescription medications. Enrollees who take multiple medications on an ongoing basis may be contacted by someone from the Medication Pathfinder Network. They will invite you to a one-on-one session with a pharmacist to review your medications. This review is  provide you with more information about the medications you are taking and determine if your use of medications is safe and appropriate for your condition(s).

If needed, the pharmacist may contact your provider and ask for additional information. If you choose to participate, both you and your provider will receive a report and summary of clinical recommendations made by your Medication Pathfinder pharmacist.

Please note that this program is free to you and is voluntary; you don't have to participate. However, it's a great way to learn more about your medications.
