Participating pharmacies in the chains listed below can provide vaccines to UMP Classic,UMP CDHP, and UMP Plus members. Contact your local pharmacy for age limitations.

Many independent pharmacies also provide vaccines. Call Washington State Rx Services at 1-888-361-1611 to ask if your local pharmacy does.
The plan also pays for vaccines received at local public health departments, but you usually have to pay up front and get reimbursed later. You must use a medical claim form to be reimbursed for a vaccine you pay for out of pocket.
TIP: If you get a vaccine from an out-of-network provider, make sure that you submit your claim to Regence BlueShield as a medical claim. Member-submitted vaccine claims sent to Washington State Rx Services will be denied.
