• You must present either a Minnesota driver’s license, identification card or instruction permit that is current or not expired more than five years if it has a color image of the applicant.
  • A Minnesota driver’s license, identification card or instruction permit that is current or not to be expired more than one year if it does not have a color image of the applicant.

If you do not have such a document you must present one primary document AND one secondary document.

Primary Documents

  • Certified copy of a birth certificate issued by a government bureau of vital statistics or board of health in the United States, District of Columbia, Guam, Puerto Rico or the Virgin Islands.
  • United States Department of State Certificate of Birth Abroad, FS-545 or DS-1350
  • United States Embassy Report of Birth Abroad of a United States Citizen, FS-240
  • Certified copy of an adoption certificate from a United States court
  • Unexpired United States Department of Defense identification card DD-2
  • Valid unexpired US passport
  • Valid unexpired passport from a country other than the US with a valid unexpired INS I-94 arrival and departure form or an unexpired I-551 stamp
  • A Canadian birth certificate or naturalization certificate with a valid I-94 form attached. Must be presented with a photo Secondary
  • Document issued by a Canadian government agency.
  • One of the following unexpired documents issued by the United State Department of Justice:
    • Certificate of Naturalization, N-550, N-570, N-578
    • Certificate of Citizenship, N-560, N-561, N-645
    • US Citizen Identification card, I-179, I-197
    • Permanent Resident or Resident Alien Card, I-551 or I-151 that is valid
    • Northern Mariana Card, I-873
    • American Indian Card, I-872
    • Employment Authorization card with photo, I-688, I-688A, or I-688B or I-766
    • Re-entry Permit/Refugee Travel Document, I-571
    • All documents are subject to verification and may not be altered.
    • Any document not in English must be accompanied by an approved English translation.
    • The name of the state-issued document will be that on the Primary document unless other documents are presented showing a change in legal name.

Secondary Documents

  • Another primary document
  • Driver’s license, identification card, or permit issued by a U.S. state other than Minnesota, by the District of Columbia, Guam, Puerto Rico, the Virgin Islands, or a Canadian province or territory, expired for more than one year but not more than five years, with a color image of the applicant.
  • Photo driver’s license, state identification card or permit issued by a U.S. state other than Minnesota, or by the District of Columbia, Guam, Puerto Rico, the Virgin Islands, or a Canadian province or territory, that is current or expired for not more than five years
  • Certified copy of a United States or Canadian court order with full legal name and date of birth
  • United States or Canadian government jurisdiction employee photo identification card
  • Certified copy of a birth certificate from a government jurisdiction other than the United States, District of Columbia, Guam, U.S. Virgin Islands, or Puerto Rico
  • Current Department of Defense identification card, DD-1173 or DD-214
  • Certified copy of applicant’s government issued marriage certificate
  • Unexpired permit to carry a firearm or concealed weapon issued with photo by a United States police department or sheriff
  • Current pilot’s license issued by the FAA
  • Certified secondary or post-secondary school transcript containing full legal name and date of birth
  • United States nonmetal, non-laminated social security card or Canadian social insurance card
  • Current secondary school student identification card with student’s name, a photograph, and date of birth or unique identification number
